SANJ O A Q U I N Environmental Health Department <br /> Complaint Record Daily Time and Activity Summary from4/19/2017 to <br /> 4/19/2019 for C00046832 located at <br /> 6440 AVIATION DR <br /> Activity Date Recorded by <br /> 06/05/2018 1:42 am-4:42 am: PGE spill PE 2546 MANZO <br /> 180 min SC 974 <br /> 1:42 am, text to Dave Larsen asking for a status update (209-401-9719), waited, <br /> no response received <br /> 2:20 am, call to Tasha with PGE (510-344-725), she indicated that she hasn't <br /> heard back from Dave but they were supposed to be starting at 1:30 am; Tasha <br /> suggested I should head out to the site to be there and that 3 am would be a good <br /> time for me to be there to verify clean up. <br /> 3:06 am onsite, can't locate the transformer, drove around. <br /> 3:18 am called Tasha, let her know that I am onsite, explained where I was. I <br /> located a transformer nearby, told her what I am seeing and that the PGE crew is <br /> not onsite. She indicated that transformer is not de-energized and to not approach <br /> it. I took several pictures from curb site. <br /> 3:29 am call to Tasha, described the transformer, told her that from where I am <br /> standing I don't see any damage or spillage on the ground. Told her that I can text <br /> her some pictures. Took pictures on ER phone, texted to her along with my <br /> location, asked her to confirm if I am at the right place. Called her back, she <br /> indicated that she got a hold of Dave ( Dave Larsen with PGE) and he will be <br /> doing the clean up but he is tight up somewhere else and she doesn't know when <br /> they will be done. The area of this spill is located when a lot of trucks are being <br /> parked along side, as I was walking around. Someone pulled up and was walking <br /> toward me, I got in the car and drove off not wanting to be approached by a male. <br /> Called Tasha explained that it doesn't seem safe and noone is here and that I will <br /> be leaving the site but will return to verify clean up. Tasha indicated that she will <br /> call me when the crew arrives onsite. <br /> 3:58 am text to Tasha asking to keep me updated. <br /> 5:39 am, text to Tasha asking if there is any update, since I didn't receive a call. <br /> Received no response <br /> 7:14 am, called Tasha asking for the status update, let her know that I haven't <br /> received any phone calls for the clean up, she indicated that she didn't receive any <br /> calls either but she will check on it. <br /> 7:38 am, called Tasha, went into the voice mail, left a message asking for a status <br /> update. <br /> 06/06/2018 Met with Dave Larson from PG&E. The spill had been cleaned up. Approximately PE 2546 ALANIZ <br /> 6 min 6 gallons of oil spilled. The accident was caused by a farmer driving equipment. SC 544 <br /> Four 55 gallon drums of waste was generated. The waste is currently labeled as <br /> hazardous waste with a pending analysis. Samples are going to be run for PCBs. <br /> Note: this is what was said for notice to abate. Notice to abate was completed on <br /> 6/6/2018 and emailed to Alex Steele. <br /> 06/06/2018 Prop 65 completed. PE 2546 ALANIZ <br /> 18 min SC 203 <br /> 06/06/2018 Entered in complaint information to complete ER record and notice to abate. PE 2546 ALANIZ <br /> 24 min Notice to abate was emailed to Alex Steele at<R2SV@pge.com>. SC 544 <br /> 06/25/2018 Completed photo log from final clean-up on 6-5-2018. PE 2546 ALANIZ <br /> 12 min SC 544 <br /> 04/05/2019 Reviewed narrative. I never received the PCB results sent an email to Alex Steele PE 2546 ALANIZ <br /> 18 min asking for them and to confirm that the waste was taken to 4040 West Ln. SC 544 <br /> Called and left a message-209-337-8902 <br /> Page 2 of 3 <br />
