The following is an itemized list of hazardous waste violations that have not been <br /> addressed for JAAS AUTO SERVICE as of April29, 2019. <br /> Open violations from November 30, 2018 inspection <br /> Violation #101 -Managed a hazardous waste without an identification number. <br /> This facility's hazardous waste ID number is inactive. A hazardous waste generator shall not treat, store, dispose <br /> of, transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC <br /> web site http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number. <br /> Immediately reactivate your state ID number and submit evidence to the EHD. <br /> Violation #103 -Failed to retain hazardous waste determination including waste analysis on site for 3 years. <br /> No waste analysis was available for the metal fines waste. A generator shall make a hazardous waste <br /> determination and keep a record of any test results, waste analyses, or other determinations made in accordance <br /> with hazardous waste regulations for at least three years from the date that the waste was last disposed of. <br /> Immediately locate a copy of the waste analysis conducted for the metal waste and submit a copy to the EHD. <br /> Violation #107 -Failed to prepare and implement a contingency plan. <br /> Facility failed to prepare a contingency plan. The owner or operator shall prepare a contingency plan designed to <br /> minimize hazards to human health or the environment from fires, explosions, or any unplanned release of <br /> hazardous waste to air, soil, or surface water and immediately implement that plan whenever there is a fire, <br /> explosion, or release of hazardous waste which could threaten human health or the environment. Immediately <br /> prepare a contingency plan and submit a copy to the EHD. <br /> Violation #114- Failed to keep signed copy of manifests from the designated facility for three years. <br /> Copies of uniform manifests for the disposal of hazardous waste from 11/30/2015-11/30/2018 were not found on <br /> site for the following waste streams: used absorbent, used oil, used antifreeze, and used paper oil filters. <br /> Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility on site for three <br /> years and have them readily available for review. Immediately locate a copy of all manifests for the last three years, <br /> maintain them on site, and submit copies to the EHD. <br /> Violation #117-Failed to complete training on hazardous waste management and emergency response <br /> procedures. <br /> According to Jeremy Bongcaron (Manager), the employees do not get hazardous waste training. In addition, he <br /> said that there is no training program in place. Personnel who handle hazardous waste shall be trained within six <br /> months of assignment and take part in an annual review of the initial training received. At a minimum, the training <br /> program shall be designed to ensure that facility personnel are able to respond effectively to emergencies by <br /> familiarizing them with emergency procedures, emergency equipment, and emergency systems, including where <br /> applicable: <br /> 1. procedures for using, inspecting, repairing, and replacing facility emergency and monitoring equipment; <br /> 2. key parameters for automatic waste feed cut-off systems; <br /> 3. communications or alarm systems; <br /> 4. response to fires or explosions; <br /> 5. response to ground-water contamination incidents; and <br /> 6. shutdown of operations. <br /> The owner or operator shall maintain the following documents and records at the facility: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job description for each position listed above; <br /> 3. written description of the type and amount of both introductory and continuing training that will be given to <br /> each person filing positions listed above; <br /> 4. records that document that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former <br /> employees shall be kept for at least three years from the date the employee last worked at the facility. Immediately <br /> provide employees with initial or refresher hazardous waste training. Submit a copy of the roster and the syllabus to <br /> the EHD. <br /> Page 1 of 17 <br />
