The following is an itemized list of hazardous waste violations that have not been <br /> addressed for JAAS AUTO SERVICE as of April29, 2019. <br /> Open violations from November 30, 2018 inspection <br /> Violation #118-Failed to maintain complete personnel training records. <br /> No employee training records for the management of hazardous waste were available for review during the <br /> inspection. Items 1-4 listed below were missing. Employee training records shall include: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job description for each position listed above; <br /> 3. written description of the type and amount of both introductory and continuing training that will be given to <br /> each person filing positions listed above; <br /> 4. records that document that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former <br /> employees shall be kept for at least three years from the date the employee last worked at the facility. Ensure that <br /> employee training is properly documented. Submit the above required information to the EHD for review. <br /> Violation #119- Failed to keep copies of consolidated manifesting receipts for three years. <br /> Copies of hazardous waste disposal records from 11/30/2015-11/30/2018 were not found on site for the following <br /> waste streams: used absorbent, used oil, used antifreeze, and used paper oil filters. Hazardous waste generators <br /> shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated <br /> manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy <br /> of all manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD. <br /> Violation #120 -Consolidated manifest receipts failed to contain complete information. <br /> The following consolidated manifest receipts were observed incomplete: <br /> -Used oil invoice 490 (8/2/2018) from Crystal Blue Environmental was observed without a generator's signature. <br /> -Used oil invoice 454 (6/17/2018) from Crystal Blue Environmental was observed without a generator's signature. <br /> All copies of each receipt shall contain all of the following information: <br /> -The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative; <br /> -The date of the shipment; <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream <br /> type listed in subdivision (c), and its proper shipping description, including the hazardous class and United <br /> Nations/North America(UN/NA) identification number, if applicable; <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, address, and identification number; <br /> -The driver's signature; <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be <br /> economically practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain <br /> all required information. <br /> Violation#121 - Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years. <br /> Bills of lading or manifests for the management of lead acid batteries for the last three years were not found on site. <br /> A copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all bills of <br /> lading or manifests for lead acid batteries for the last three years, maintain them on site, and submit copies to the <br /> EHD. <br /> Page 2 of 17 <br />
