The following is an itemized list of hazardous waste violations that have not been <br /> addressed for JAAS AUTO SERVICE as of Apri129, 2019. <br /> Open violations from November 30, 2018 inspection <br /> Violation #123 - Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on 8/2/2016 and an inspection report was issued identifying information to be submitted <br /> to bring this site into compliance. This information was required to be submitted by 9/1/2016. This information has <br /> not been received resulting in a non-compliant status for this facility. An operator that receives an inspection report <br /> shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or <br /> proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions <br /> taken or proposed is submitted within 30 days of receiving an inspection report. <br /> Violation #207 -Failure to properly manage reusable soiled textile materials. <br /> Receipts for the soiled rags for the last three years were not available for review during the inspection. The receipts <br /> for laundered rags are required to be kept for three years. Please provide some copies of the 2018 records to the <br /> EHD for review. <br /> Violation #301 - Facility not maintained to minimize the release of a hazardous waste. <br /> According to Sopheak San, the metal shavings are added to the absorbent drum. Metal shavings were observed <br /> pooling on the floor and on the equipment. Sopheak San said that his waste needed to be picked up and that all of <br /> his containers were getting full. Several 5 gallon buckets filled with paper oil filters (overfilled) were observed in the <br /> used oil tank area. Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or <br /> release of hazardous waste to air, soil, or surface water which could threaten human health or the environment <br /> Immediately clean the metal shavings area, and manage according to Title 22 hazardous waste regulations. In <br /> addition, properly secure the paper oil filters to prevent a release. Submit a statement and supporting <br /> documentation explaining how this waste was managed. <br /> Violation #403 - Failed to keep hazardous waste containers closed except when adding or removing <br /> hazardous waste. <br /> In the used oil storage area: <br /> -Five 5 gallon plastic buckets containing used paper oil filters were observed without a lid. <br /> -The 500 gallon used oil tank was full and open at the top. <br /> In the pit area: <br /> -One 5 gallon bucket containing paper oil filters was observed without a lid. <br /> All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately <br /> close these containers and ensure all hazardous waste containers are closed when not adding or removing waste. <br /> Violation #406 -Contaminated container not marked with date emptied or managed within one year of being <br /> emptied. <br /> One black 55 gallon drum was observed without an empty label. Containers larger than five gallons that previously <br /> held a hazardous material shall be marked with the date they were emptied and managed within one year of being <br /> emptied. Immediately mark and manage all empty containers per this section. <br /> Violation #501 - Failed to completely label stationary hazardous waste tanks. <br /> The used oil tank label had an illegible accumulation start date. All hazardous waste tanks shall be marked with the <br /> words "Hazardous Waste" and the accumulation start date. Immediately ensure that all hazardous waste tanks are <br /> marked with the required information. <br /> Violation #502 -Failed to comply with new hazardous waste tank system assessment requirements. <br /> Facility does not have a tank assessment for the 500 gallon used oil tank. The owner or operator of a hazardous <br /> waste tank shall obtain a written assessment reviewed and certified by an independent, qualified, professional <br /> engineer, registered in California attesting that the system has sufficient structural integrity, is acceptable for the <br /> transferring, storing, and treating of hazardous waste, and that the tanks and containment system are suitably <br /> designed. Immediately have an assessment prepared for the 500 gallon used oil tank. Submit a schedule showing <br /> when this will be accomplished to the EHD and submit a copy of the assessment to the EHD for review. <br /> Page 3 of 17 <br />
