Aris Cacapit [EH]
From: Eric Janzen [Eric@ablemaintenance.com]
Sent: Tuesday, November 30, 2010 9:40 AM
To: Aris Cacapit[EH]
Subject: RE: certifications: Able Maintenance, Inc. RE. Tesoro/USA stations in Lockeford & Lodi
Attachments: order.form.bravo.pdf; SBRAVO-Mike.pdf; ICC-Mike.pdf
Aris,
I've attached copies of the order forms for the dispenser shields/frames.
I have also attached copies of the Bravo Gold pocket card issued the to Mike Trejo and Mike's ICC certification for
your reference. Rod Brownlee and Asa Heintz also hold current certifications but are not expected to be involved
in field activities.
I am a little concerned regarding your request for Bravo Gold certifications issued to the individual members of our
staff. The"Gold" classification means that our Contractor's License (issued to Rod Brownlee-President) meets
Bravo's requirements. The individual staff training, for which pocket cards are issued, concerns installation of
UDC's and containment testing procedures. Our proposed scope-of-work does not include either of these
activities. So I just want to confirm that we are not proposing replacement or modification of the existing UDC's.
The adapter is an above-grade accessory which bolts into place using the original factory mounting locations.
Installation will not affect any below-grade containment and will not require testing and re-certification of the UDC.
Is it your intent to require containment testing of the UDC's regardless?
Eric Janzen
Able Maintenance, Inc.
From: Aris Cacapit [EH] [mailto:acacapit@sjcehd.com]
Sent: Monday, November 29, 2010 4:39 PM
To: Eric Janzen
Subject: RE: certifications: Able Maintenance, Inc. RE. Tesoro/USA stations in Lockeford & Lodi
Hi Eric,
Please find attached a link to the Bravo Website for the conversion frame order form. It is a completed copy of this
form that your company has sent to Bravo that we would like a copy of.
http://www.sbravo.com/documents/OrderForm Con_v_Fr_m..pdf
The technician(s)that completed the Bravo Gold Certification training, in addition to Rod Brownlee should have
also received individual bravo gold certification cards too right? Please send a copy of that individual's bravo gold
certification.
Thank you,
Aris Cacapit, REHS
San Joaquin County Environmental Health Department
600 E.
