AN Environmental Health Department <br /> QU <br /> COUNTY— <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Aramark Uniform & Career Apparel, 7679 LONGEST, STOCKTON April 04, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The map does not show loading areas, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, the hazardous materials handling and storage areas (compressed <br /> gases and outside waste storage on the north and west sides of the facility), and a legend that defines all of the <br /> codes/numbers used on the map. A site map shall contain a north orientation, loading areas, internal roads, <br /> adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, <br /> hazardous material handling and storage areas, and emergency response equipment. If a site map element is not <br /> applicable for your facility then list it on the map and label as"NA". Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, <br /> and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 8 CCR 2658, 2659 Failed to submit a training program or program is not reasonable or adequate for facility. <br /> The training program lacks training of the immediate notification contacts to the appropriate local emergency repose <br /> personnel and the Environmental Health Department. Training programs for all employees and annual training, <br /> including refresher courses, shall include safety procedures for the event of a release or threatened release of a <br /> hazardous material, including, but not limited to, familiarity with the facility's emergency response plans and <br /> procedures. These training programs may take into consideration of each employee. Immediately prepare or <br /> update the facility's training program, log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually by January 15 for 2018 and 2109. The hazardous materials inventory shall be submitted <br /> by January 15 of each calendar year and may be submitted beginning November 1 of the previous year. <br /> Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to <br /> the EHD for approval. <br /> This is a Class II violation. <br /> 12 HSC 25505(c) Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section.
