Employees undergo safety training throughout the year at monthly meetings which address site <br /> specific issues and concerns. Our safety business partner Health and Safety Institute, which is a <br /> web based training program employees are enrolled in industry topics and classes for <br /> knowledge base education. The annual training course work for employees are: <br /> Emergency Preparedness <br /> Hazardous Waste <br /> Lockout/Tagout: Affected Employees <br /> Lockout/Tagout: Authorized Employees <br /> Bloodborne Pathogens <br /> GHS Globalize Your Communication <br /> Safety Orientation <br /> Fire Extinguisher <br /> Forklift Safety: Basics and Best Practices <br /> Overhead Crane Safety <br /> Personal Protective Equipment: Your Final Defense <br /> Sexual Harassment: Understanding and Preventing <br /> Workplace Violence, Prevent the Threat <br /> Item #10. Sumiden Wire is still on plan to have a Principle Engineer from <br /> Chemical Data Management Systems review our SWPPP and SPCCP. Once plans are <br /> updated the revised map will be submitted. Current submittal is available to review for <br /> approval in CERS. <br /> • Contingency Plan corrected on site using CERS provided template titled "Consolidated <br /> Emergency Response / Contingency Plan." Additionally, emergency response numbers <br /> have been updated in Sumiden Document held on site with CERS document inserted as <br /> addendum <br /> Item #11. HSC 25505 (a)(4) Failed to provide or document initial and/or refresher training to <br /> appropriate personnel. Sumiden Wire uses the following document to provide training for its <br /> hazardous material handler document titled, "Hazardous Material Handler Training RCRA & <br /> NON-RCRA as required by Title 22 of the California Code of Regulations 66265.16." This training <br /> document was originally drafted on 10/16/2006 by WFM (Wayne Mannor). Attached is the <br /> document revised 5/15/2019 and training records of those trained in the subject.
