AN Environmental Health Department <br /> QU <br /> COUNTY— <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: I Facility Address: Date: <br /> SUMIDEN WIRE PRODUCTS CORPORATION 1412 EL PINAL DR, STOCKTON April 23, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 10 HSC 25508.1(a)-(f) Failed to submit revisions to business plan within 30 days of substantial change in operations. <br /> During the inspection two 55 gallon drums of Chemtreat CL-49, seven 55 gallon drums of Acid Aid LG10-NF, eight <br /> 400 pound drums of Vicafil TSWCB, at least eighty-four 400 pound drums of TNA 1610 PC, at least 192 boxes of <br /> epoxy powder coat (44 pounds each), one 55 gallon drum of Bio-Circle L and one 55 gallon drum of Chemtreat BL <br /> 8700 were found on site and the updated information has not been submitted. Within 30 days of one the following <br /> events, business shall electronically update the information submitted to the California Environmental Reporting <br /> System(CERS): <br /> -A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership, or business name. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records for were not available at the time of inspection. The business plan shall include <br /> provisions for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br /> provide employee training for appropriate personnel and submit a copy of the training records to the EHD. <br /> This is a Class II violation. <br /> 15 HSC 25510(a) Failed to report actual or threatened release to the EHD and/or CalOES Warning Center. <br /> A release of sulfuric acid occurred on February 20, 2019 and was not reported to the EHD and CalOES. A release <br /> or threatened release of a hazardous material shall be verbally reported immediately to the EHD and CalOES. <br /> Ensure that any future releases or threatened releases of a hazardous material are verbally reported immediately to <br /> the EHD and CalOES. <br /> This is a Class I violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the completed inspection report. <br /> Facility does not have internal roads and this element was not addressed on the site map. Please address this <br /> by either writing "NA" for not applicable or a statement that says the element is not on site.
