Do these satisfy that our staff was trained for Cot for the violation from
2017?
Also attached to this email is Public Health Services "Department Emergency Plan" and "Facility Threat Plan—
Hazelton". Please review to see if this satisfies the emergency plan for employee training of contacting emergency
responders and evacuation procedures, which are the procedures employees are expected to follow upon any threat
with Diesel Fuel No. 2 and Cot. Both of these documents have been submitted and accepted by San Joaquin County
Office of Emergency Services and Human Resources Risk Management.
Please respond via "Reply All"to this email as I want to ensure clear communication while resolving this issue. Thanks in
advance for any guidance you can provide.
--wMehssa Yago
209-468-9874
From: Melissa Yago [PHS]
Sent:Tuesday, April 30, 2019 4:06 PM
To: Robert Lopez [EH] <rlopez@sjcehd.com>
Cc: Bruce Cosby [PHS] <bcosby@sjcphs.org>; Muniappa Naidu [EH] <Mnaidu@sjcehd.com>
Subject: Public Health CERS 10745503 Emp Training Documents
Importance: High
Mr. Lopez,
Attached are two items in response to our Employee Training Plan violation in handling hazardous materials at
Public Health Services.
PHS EmpTraining Diesel letter.pdf details how training for Diesel Fuel No. 2 has not been able to occur
recently, due to the sole employee needing the training is currently on medical leave.
1
