PHS EmpTraining CO2—2-22-2018.pdf contains the sign-in sheet of employees who are required to be trained
and the handouts discussed during the training.
As stated in my submission dated April 18, 2019, our Employee Training Plan includes:
"Employees who directly handle hazardous materials and wastes in day-to-day operations are those working in the
Laboratory. New hires are trained immediately in the Laboratory Bio-Safety Plan, Procedure for Donning and Doffing PPE
for Working with Select Agents, Chemical Hygiene Plan, among other practices. This training is continuous as these
training topics are addressed at monthly unit staff meetings."
Furthermore, the response to an emergency involving either one of these hazardous items is Evacuation and
calling for Emergency Personnel. As deemed necessary in our Illness and Injury Prevention Program and San
Joaquin County Office of Emergency Services, all PHS employees are trained annually in emergency
evacuations and reporting emergency situations by calling 911; the procedures are detailed in our Department
Emergency Plan and Facility Threat Plan: Hazelton. Our most recent evacuation training exercise was April 24,
2019, an attendance list can be provided if necessary.
I would be happy to answer any questions or concerns you may have. Please "Reply to All" with any issues.
Sincerely,
wleftssa yago
Management Analyst II 1 MAA-TCM Coordinator
IS&Son iooquin CQvnty
Public Health Services
health grows hose
1601 E Hazelton Avenue I Stockton, CA 95205 1 Phone: 209-468-9874 1 Fax: 209-468-3823
2
