The designated operator failed to document all the alarms from the attached alarm history on the February 2018 designated operator monthly inspection report and failed to check that they were responded to appropriately. The 1/24/18 91 STP sump fuel alarm was documented, but the designated operator did not verify that the alarm was properly responded to and did not document that an inspection of the sump was conducted. At the time of inspection the 91 STP sump was observed to contain over 50 gallons of liquid and the sensor was on its side, not in alarm and unable to detect a leak. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a Class II violation. 211 CCR 2715(f)(2) Designated operator employee training not performed or log not kept. The designated operator employee training for Erica Romero was last performed on 1/16/17 and was not current. Ms. Romero was the only employee on site at the start of inspection. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the updated training records to the EHD. This is a Class II violation. 302 HSC 25291(b) UST system incapable of detecting leak from primary into secondary containment(pre-Jul 2003). The 91 STP sump sensor was laying on its side in over 50 gallons of liquid and not located to detect a leak at the earliest opportunity. The sensor passed functionality testing during the monitoring certification. The monitoring system must be capable of detecting a leak from primary containment into secondary containment at all times. The sensor was relocated upright to the lowest point of the sump by the service technician. Ensure that all monitoring equipment is maintained to detect a leak at the earliest opportunity.
