JOAQUIN <br /> —COUNTY <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: FA , <br /> FAMILY DOLLAR#31867 2318 S AIRPORT WAY STOCKTON 25 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 102 CCR 66262.11 Failed to determine if a waste is a hazardous waste. <br /> -A package containing what appeared to be six Boost meal replacement shakes and another package containing <br /> EAS Advant EDGE High Protein shakes were found in the facility dumpster. Boost meal replacement shakes were <br /> found on a sales shelf next to Slim Fast Original meal replacement shakes, both of which had Nutrition Facts labels, <br /> inside the facility. The EAS Advant EDGE shakes also had Nutrition Facts labels. Slim Fast Original meal <br /> replacement shakes were observed bagged and in a Toxic&OTC(over-the-counter)Pharmaceuticals(TX OTC) <br /> hazardous waste(HW)tote. (see photos IMG 5593-4, 5607, 5611-2) <br /> - A container of Suavitel fabric softener was found leaking from a broken cap adjacent to the facility mop sink and was <br /> not labeled as a store-use item (IMG 5585-7). Monica Gabales, store manager, did not know how long the container <br /> had been there. Monica placed the fabric softener in front of the HW storage area to be processed as hazardous <br /> waste. <br /> Any person who generates a waste shall determine if the waste is a hazardous waste. <br /> Immediately stop the disposal of Boost meal replacement shakes and EAS Advant EDGE High Protein shakes into <br /> the trash, make a hazardous waste determination for these waste items and for the Suavitel fabric softener, and <br /> manage them according the Title 22 hazardous waste regulations. <br /> Submit a statement and supporting documentation explaining how this waste was managed. <br /> This is a Class II violation. <br /> FA0001729 PRO544373 SCO01 04/25/2019 <br /> EHD 22-01 Rev.09/27/2018 Page 4 of 16 Small Quantity Hazardous Waste Generator OR <br /> 1868 E.
