SAN <br /> J O A f L Q U I N Environmental Health Department <br /> ti T 1f <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> FAMILY DOLLAR#31867 2318 S AIRPORT WAY STOCKTON Aril 25 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) <br /> handling or the training provided has been inadequate. <br /> -Two bags containing several boxes of TNT Pop-Its Snappers,one of which was labeled with the words"Hazardous <br /> Waste,"were found on the floor in front of the HW storage area(IMG 5542, 5544-5). Monica Gabales, store manager, <br /> did not know who placed the items into the HW bag, nor why the items were placed in the bag.When Monica <br /> scanned a box of TNT Pop-It Snappers, the hand-held scanner stated"SKU Price is 0.00 or 0.01, SKUS with these <br /> prices are not allowed in this transaction"(IMG 5552). <br /> -An unlabeled tote was found in the HW storage area containing the following bagged items(IMG 5546-50): <br /> -Three prayer candles labeled"ST." <br /> -A Wireless Gear Wireless FM Transmitter With Remote labeled"UW."Monica labeled the tote with a pre-printed <br /> Universal Waste(UW)label. <br /> -A State Regulated(ST)HW tote was found with accumulation start date 9/10/18,which is past the 180 day storage <br /> limit, and contained the following bagged items(IMG 5554): <br /> -Three empty containers of motor oil,which were indicated by the hand-held scanner to be placed into a TX OTC <br /> HW tote(IMG 5555-6). However,the ST HW tote pre-printed label had"Motor Oil"marked as the contents.When <br /> asked how employees determine which HW tote an item should be placed into, Monica stated employees follow what <br /> is indicated on the scanner, but she suspected whoever placed the motor oil containers into the ST HW tote followed <br /> the tote label, rather than following the scanner. Monica stated the empty motor oil containers were left by customers <br /> who buy and use the motor oil, but leave the containers in the facility's parking lot. <br /> -A container of LA's Totally Awesome All Purpose Cleaner Degreaser Spot Remover was found open, <br /> singled-bagged, and leaking into the bottom of the ST HW tote(IMG 5557, 5561-2).Although, according to the Family <br /> Dollar Hazardous Waste Training Guide, absorbent should be applied to free liquids and leaking items should be <br /> double bagged (IMG 5625, 5628). <br /> -A bottle of Tylenol Extra Strength tablets,which the hand-held scanner indicated should be placed in the ST HW <br /> tote. Although, according to the Family Dollar StrongPak poster posted adjacent the HW storage area, OTC <br /> medicines are to be placed in a TX OTC HW tote(IMG 5563-4, 5543, 5582). <br /> -An almost empty container of Magic Power Cleaner with Bleach was found labeled"ST,"and which the hand-held <br /> scanner indicated should be placed in the ST HW tote.Although, according to the Family Dollar StrongPak Poster, <br /> items containing bleach are classified as oxidizers(OX)(IMG 5567-70). <br /> -Used absorbent was not found at the time of inspection to account for the missing liquid from the Magic Power <br /> Cleaner with Bleach container. Monica did not know what happened to the missing liquid that was once in the <br /> container.According to Gloria Oros,district manager,the cleaner may have been a"store-use"item used by <br /> employees to clean the facility.When asked how employees know if an item is a"store-use"item, Gloria stated the <br /> item be marked with the words"store-use,"and/or a sticker will be placed on the item's barcode,when removed from <br /> the store's inventory.The Magic Power Cleaner with Bleach container was not marked with the words"store-use,"nor <br /> was there a sticker on the item's barcode. <br /> FA0001729 PRO544373 SC001 04/25/2019 <br /> EHD 22-01 Rev.09/27/2018 Page 5 of 16 Small Quantity Hazardous Waste Generator OIR <br /> 1868 E.
