The most current Certification of Financial Responsibility and Letter from Chief Financial Officer are dated 11/21/14 Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. This is a minor violation. 104 CCR 2711 UST Operating Permit Application for Facility and Tank information not submitted or current. UST Facility Operating Permit Application and UST Tank Information forms for the 87 and 91 product USTs are not current in CERS. The Business Owner/Operator Identification form and Facility form are lacking the name and title of the Primary Emergency Contact; and the title of the Name of Signer, Casey Paoletti, needs to be added to the Business Owner/Operator Identification. The UST Facility Operating Permit Application needs the Tank Operator's name and phone number; the tank owner type should be changed from County Agency to Non-government; the Financial Responsibility Mechanism should only have one mechanism so change Insurance to"No", and the Applicant Name, Casey Poaletti, is lacking Casey's title. Tank ID#TA0515604 and TA0515605 need the following corrections: The Fill Components Installed should have"yes"for Spill Bucket and Striker Plate/Bottom Protector; confirm whether the Vent Primary Containment is steel or fiberglass, and the Applicant Certification needs the current person's name and title(Contact is not an appropriate title.) Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a Class II violation. 106 CCR 2632 Monitoring and response plans not current or approved by the EHD. The response plan is not current and/or not approved by the EHD and has not been uploaded to California Environmental Reporting System (CERS). The response plan must be uploaded to the CERS. Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. The monitoring plan is not current and/or not approved by the EHD. The Leak Sensor Model number should be corrected to VR794390-460, not 420; the automatic tank gauging information needs to be provided; Periodic System Testing should have"yes"for Secondary Containment Testing; and the Personnel Responsibilities should name the correct person and provide the person's title, Contact is not a title. Under Tank Monitoring is Performed Using the Following Methods, enter"yes"for continuous electronic tank monitoring. Also update the Applicant Certification name and title. The monitoring plan must be uploaded to the California Environmental Reporting System(CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD.
