Based on the results of this assessment work,
a number of additional soil borings, monitoring wells, and/or vapor wells will need to be installed to
complement, verify and monitor the assessment work data.
Therefore, please submit permit applications for each separate parcel to be drilled on, a copy of the access
agreement for each off site parcel, and the $89.00 permit application fee for each permit to PHSIEHD by
October 17, 1994.
The wells at this site must be sampled, and the results submitted, to PHS/EHD on a regular quarterly basis.
PHS/EHD requires 48 hours advance notice prior to the sampling event so that a representative can be available
to observe.
In addition, the free product found in monitoring well MW#3 must be removed to the maximum extent
practicable. Please submit a workplan to describe a procedure to properly remove, handle, store and
dispose of the free product to PHS/EHD within 14 days from the date of this letter.
If you have any questions or wish to discuss this letter in more detail, please contact Linda Turkatte, Senior
REHS, at (209) 468-3441.
Donna Heran, REHS, Director
Environmental Health Division
T
Linda Turkatte, Senior REHS Diane M.
