All copies of each receipt shall <br /> contain all of the following information: <br /> -The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative; <br /> -The date of the shipment; <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream <br /> type listed in subdivision (c), and its proper shipping description, including the hazardous class and United <br /> Nations/North America (UN/NA) identification number, if applicable; <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, address, and identification number; <br /> -The driver's signature; <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be <br /> economically practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain all <br /> required information. <br /> This is a Class II violation. <br /> 117 HSC 25404(e)(4) Failed to report program data electronically. <br /> A submission to the California Electronic Reporting System (CERS) by the current business owner for the hazardous <br /> waste program has not been made. Beginning January 1, 2013, all businesses are required to submit all new(or any <br /> changes to existing) hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to <br /> include your hazardous waste activity in the Businesses Activities section in CERS in addition to any other relevant <br /> activities and required fields. <br /> This is a Class II violation. <br /> 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. <br /> Dark oil stains and oily debris were found outside underneath the air compressor next to the half full quart bottle of <br /> Volvo Penta full synthetic oil. Facilities shall be maintained and operated to minimize the possibility of a fire, <br /> explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the <br /> environment. Immediately clean up the dark oil stains and oily debris and manage according to Title 22 hazardous <br /> waste regulations.
