T <br />t <br />g. If rejected, a brief explanation of why the waste <br />was rejected; <br />h. A brief explanation of why the waste was <br />unmanifested, if known; and <br />i. A certification as required by item II.H of this <br />permit. <br />5. Used oil shall be manifested under either one of the <br />following procedures: <br />a. The procedures prescribed in conditions III.G.1, <br />III.G.2, III.G.3, and III.G.4; or <br />b. The following modified manifesting procedures may <br />be used by a registered hazardous waste hauler <br />and shall be used only with the consent of the <br />generator: <br />(1) A separate manifest shall be completed by <br />each vehicle driver, with respect to each <br />transport vehicle operated by that driver <br />for each date. <br />(2) An EPA identification number is not required <br />for an individual generator. <br />(3) The hauler shall complete both the <br />generator's and the hauler's sections of the <br />manifest using the hauler's name. The <br />hauler's section shall be completed prior to <br />commencing each day's used oil collections. <br />The driver shall sign and date the <br />generator's and the hauler's sections of the <br />manifest. <br />(4) The hauler's name, EPA identification <br />number, and terminal address shall be <br />entered in the generator's name, address, <br />and EPA identification number spaces of the <br />manifest. <br />(5) The hauler shall attach to the front of the <br />manifest legible receipts for each quantity <br />of used oil that is received from a <br />generator. The receipts shall be used to <br />determine the total volume of used oil in <br />the vehicle. After the used oil is <br />delivered, the receipts shall be affixed to <br />the hauler's copy of the manifest. The <br />hauler shall leave a copy of the receipt <br />with the generator of the used oil. The <br />generator shall retain each receipt for at <br />least three years. <br />-17- <br />
