3 <br />s <br />(6) All closure cost estimates; and <br />(7) All waste minimization certifications. <br />b. When the owner or operator receives hazardous <br />waste from an off-site source, he must inform the <br />generator in writing that he has the appropriate <br />permit(s) for, and will accept, the waste the <br />generator is shipping. The owner or operator <br />shall keep a copy of this written notice as part <br />of the operating record. <br />Reporting and Notification Requirements <br />a. All reports and information requested by the <br />Department shall satisfy the signatory <br />requirements in item II.H. The waste <br />minimization certifications as required in item <br />II.J shall be signed in accordance with item <br />II.H. <br />b. Annual Report <br />The owner or operator shall prepare and submit <br />one copy of an annual report to the Department <br />and one copy to the appropriate Regional Water <br />Quality Control Board by March 1 of each year, <br />beginning March 1, 1990. The annual report shall <br />cover facility activities during the previous <br />calendar year and shall include the following <br />information: <br />(1) The EPA identification number, name, and <br />address of the facility; <br />(2) The calendar year covered by the report; <br />(3) Updated closure cost estimate for the <br />facility; <br />(4) The EPA identification number of each <br />hazardous waste generator from which the <br />facility received a hazardous waste during <br />the year; for imported shipment, the report <br />shall sive the name and address of the <br />foreign generator; and <br />(5) The description, quantity, and method of <br />treatment, storage, and/or disposal of each <br />hazardous waste the facility received during <br />the year, listed by the EPA identification <br />number of each generator. <br />W&M <br />
