(209)468-3433 Web:www.sigov.orglehd/unitiii.html <br /> CONTINUATION FORM Page: 3 of 5 <br /> OFFICIAL INSPECTION REPORT Date: 5/18/2010 <br /> Facility Address: 6540 Austin Rd Program: HW <br /> SUMMARY OF VIOLATIONS <br /> (CLASS 1,CLASS II,or MINOR-Notice to Comply) <br /> All Violations are currently minor. <br /> #7 Failed to determine if a waste is a hazardous waste: <br /> At time of inspection, 1 yellow 30 gallon drum approximately half full of red liquid, with rags and plastic <br /> bottles was seen near the 2-10,000 gallon red diesel aboveground storage tanks. This container did not <br /> have a label and the contents were unknown. Immediately make a hazardous waste determination <br /> pursuant to Title 22 hazardous waste regulations and handle accordingly or handle as hazardous waste. <br /> Within 30 days (June, 19, 2010) provide written documentation describing how this waste was handled. <br /> #18 Stored hazardous waste onsite longer than 90 days without permit or authorization: <br /> #20 Failed to completely label containers of hazardous waste: <br /> At time of inspection the following containers were seen improperly labeled: <br /> 1-30 gallon drum of waste absobent, missing start date & hazardous property <br /> 1-30 gallon drum of waste aerosol cans, missing start date <br /> 1-30 gallon drum of waste antifreeze, missing start date <br /> All containers of hazardous waste must be labeled at all times with the following information: <br /> 1-Words "Hazardous Waste" <br /> 2-Generator name & address <br /> 3-Contents <br /> 4-Hazardous Property <br /> 5-Physical State <br /> 6-Start Accumulation date <br /> Immediately properly label all containers of hazardous waste. Because the above mentioned containers <br /> did not have a start accumulation date, it can not be determined how long the hazardous waste has <br /> been onsite. Immediately have the hazardous waste removed by a licensed hazardous waste hauler <br /> under manifest. Within 30 days (June 19, 2010) provide this office with a copy of the hazardous waste <br /> manifest. <br /> #49 Personnel training records incomplete: <br /> At time of inspection facility personnel attend an initial hazardous waste training session upon hire. The <br /> employees repeat this training session every 3 years, with a refresher given annually. Upon inspection <br /> written documentation of the annual refresher could not be provided. The last documentated training <br /> was given in 2008. At a minimum, the training program shall be designed to ensure that facility <br /> personnel are able to respond effectively to emergencies by familiarizing them with emergency <br /> procedures, emergency equipment, and emergency systems, including where applicable, <br /> ALL E H D STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br /> EHD Inspect a Received By: Title: <br />
