Facility personnel shall take part in an annual review of the initial training received. The owner/operator shall maintain the following documents and records at the facility.- (1) acility:(1) the job title for each position at the facility related to hazardous waste management, and the name of each employee filling each job; (2) written job description for each position listed in (1) above; (3) written description of the type and amount of both introductory and continuing training that will be given to each person filing positions listed in (1) above; (4) records that document that the training has been given to and completed by facility personnel. Training records on current personnel shall be kept until closure of the facility. Training records on former employees shall be kept for at least three years from the date the employee last worked at the facility. Immediately provide employees with initial or refresher hazardous waste training. Within 30 days (June 19, 2010) provide this office with a copy of the roster and the syllabus which meet the requirements of 66265.16 CCR (California Code of Regulations). #60 Failed to keep signed copy for 3 years. #61 Failed to determine the status of hazardous waste when manifest copy not received. #62 Failed to file an exception report. At time of inspection generator copy of manifest# 000585316SKS, 1/25/08 was seen without the TSDF copy. Hazardous waste generators must maintain copies of TSDF-signed manifests for 3 years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If within 45 days, from the date that the hazardous waste was accepted by the initial transporter,the generator did not receive a copy of the manifest with the handwritten signature of the owner/operator of the designated facility, the generator shall submit to DTSC a legible copy of the manifest, with some indication that the generator has not received confirmation of delivery. Within 30 days (June 19, 2010) provide the office with a copy of the TSDF copy or the exception report submitted to DTSC. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115).
