No maintenance record was available for <br /> this repair. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and <br /> time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any <br /> visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or <br /> other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br /> reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on <br /> site and submit copies to the EHD. Locate and submit to the EHD the missing maintenance record for the 89-octane <br /> turbine motor replacement. <br /> This is a Class II violation. <br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator listed the L-6 alarm which occurred on 11-2-15 as L-6 alarm which occurred on 9-2-15 on the <br /> 11-11-15 designated operator monthly inspection report. Also, the designated operator failed to document all the <br /> alarms from the attached alarm history on the 3-17-15 designated operator monthly inspection report; the missing <br /> alarms include all the monitoring system alarms which occurred on 3-17-15. During the monthly inspection, the <br /> designated operator shall review the alarm history for the previous month, check that each alarm was documented and <br /> responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br /> alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are <br /> including all of the required information on the reports. <br /> This is a repeat violation, Class Il. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The 89-octane and 91-octane fill sump position sensitive sensors were located inside permanency fixed, holding <br /> (fiberglass) sleeves, located approximately 3-4 inches up the sump saddle and off the lowest points of their respective <br /> sumps, and not located to detect a leak at the earliest opportunity. The technician onsite stated that he can't <br /> reposition these sensors to the lowest points of their sumps, because they will be outside their holding sleeves and <br /> since they are position sensitive sensors if they were to fall over it will activate a sensor out alarm, also he asked if <br /> they fall over who will be paying for that service call. Monitoring equipment shall be maintained to be able to detect a <br /> leak at the earliest possible opportunity. The sensor was relocated to the lowest point of the sump by the service <br /> technician. Ensure that all monitoring equipment is maintained to detect a leak at the earliest opportunity. Submit <br /> proof of correction to the EHD. <br /> This is a Class II violation. <br /> Page 4 of 5 <br />
