SANJ O A Q U I N Environmental Health Department <br /> Complaint Record Daily Time and Activity Summary from 5/13/2019 to <br /> 5/13/2019 for COLLW66CY located at <br /> 6425 N Pacific Ave <br /> Activity Date Recorded by <br /> 05/13/2019 Received spill report 19-3086. On 5/12/2019, at approximately 7:30 p.m., a 3-4 PE 2546 ALANIZ <br /> 210 min gallon release of regular gasoline occurred at 5425 N Pacific Ave. Called the SC 544 <br /> Safeway fuel station at(209) 472-8600 (8:52 a.m.)and spoke with the assistant <br /> manager, Karla Carrillo. She said that she was not there for the spill but that it <br /> was cleaned up using the absorbent pads. She was not able to tell me any more <br /> information. Called the environmental contact in CERS, Ron Lee, at(925) <br /> 469-7513 (8:56 a.m.). A message was left for him. Called the assistant sales <br /> manager(emergency contact in CERS)Aldo Basurto, at(925) 580-8131 (8:58 <br /> a.m.). He informed me that the spill occurred at dispenser 5/6. A customer was <br /> pumping diesel fuel when the 87 meter broke. The release tripped the 208 sensor <br /> in the UDC and some fuel was released onto the cement pad. The spill was <br /> contained to the cement pad and did not reach any storm drains. <br /> Discussed with Garrett Backus. Made a complaint and left for the site. Arrived at <br /> 10:00 a.m. Met with Aldo and Karla. The spill was cleaned up using <br /> approximately 19 absorbent pads. The waste was put into one black 55 gallon <br /> steel drum to be disposed of as hazardous waste. No fuel was observed at the <br /> storm drain nearest to the dispenser or in the 5/6 UDC. The shear valves in the <br /> UDC had been "tripped"to prevent further fuel from leaking. A work order was <br /> placed with Service Station Systems, Inc to correct the spill. Emailed the notice to <br /> abate to Aldo at<aldo.basurto@safeway.com> and confirmed he received it. <br /> Notice to abate was also emailed to Ron at<ron.lee4@safeway.com>. Left the <br /> site at approximately 10:42 a.m. and arrived back to the EHD at 11:00 a.m. <br /> Got back to the EHD, created a photo log, updated Elena Manzo, completed a <br /> Prop 65, created a site map, completed ER record and abated the complaint. <br /> Called Aldo and informed him that the complaint would be closed. <br /> Cross-referenced to PR0513740. <br /> Page 1 of 1 <br />
