Contact Fire Department for permit. <br /> 2. All new submittals supercede the previous ones. <br /> 3. Submit monitoring and response plan to San Joaquin County Environmental Health <br /> Division (EHD)prior to scheduling tank setting appointment. <br /> 4. Submit certification of training issued by tank and piping manufacturer to EHD prior <br /> to scheduling tank setting appointment. <br /> 5. All monitoring equipment, including leak detectors, shall be listed in LG 113-14. <br /> 6. Automatic pump shutdown shall satisfy all conditions listed in Section 2636(g) of <br /> California Code of Regulation, Title 23 (CCR). <br /> 7. If any future secondary containment test as required by SB 989 fails as a result of flex <br /> boots, coated steel clamps and/or Vulkem sealant failure, the secondary containment <br /> for flex hoses shall be replaced with an approved type. A new EHD permit will be <br /> required. Note: According to Bravo representative, coated clamps and Vulkem <br /> sealant are designed for upgrading the existing Bravo system. <br /> 8. This approval does not exempt this site from any future requirement. <br /> 9. UST shall be anchored per buoyancy calculation. UST anchoring shall be installed in <br /> accordance with tank manufacturer instruction. <br /> 10. Submit as-built plan to EHD prior to scheduling final inspection. As-built plan shall <br /> be drawn to scale and include all equipment and diagram of site plan, piping layout <br /> and UST anchoring. <br /> 11. See conditions of approval for permit to install piping for additional conditions. <br /> 12. Contact Michael Kith for inspection. See attachment for the required inspections. <br /> 13. All appointment shall be scheduled 48 hours in advance. <br /> 14. Any changes in information provided to EHD shall submitted to EHD for approval <br /> prior to commencing work. <br /> 15.
