Craig, Inc. <br /> From: Michael Kith, Sr. REHS <br /> San Joaquin County <br /> Environmental Health Division <br /> Re: Application for permit to install underground storage tank at Mason Center LLC, <br /> 6425 Pacific Ave., Stockton, Ca. <br /> Mr. Hardister, <br /> After reviewing the submitted materials, I observed the following deficiencies and <br /> discrepancies: <br /> 1. Line leak detector information. The old application showed leak detector, the new <br /> one showed none. Please provide accurate information and cut-sheet on line leak <br /> detector. <br /> 2. Annual line tightness test or automatic pump shut down. The old application chose <br /> one, the new one chose none. Please choose one (page 8 of applicatiion). <br /> 3. The monitoring system. The application indicated the Veeder Root system, the cut- <br /> sheet indicated the Gilbarco. Please provide accurate information and cut-sheet on <br /> monitoring system. If Gilbarco is chosen, ensure that all sensors to be installed are on <br /> LG113-14. <br /> 4. Flex boot. On 1/25/01, you submitted an addendum to change the flex boot to Bravo <br /> BFH-3-30FRP-AS and Bravo BFH-3-40FRP-AS. Those boots were not UL- or <br /> ULC-listed or third-party approved. They do not meet the requirement of section <br /> 2636 (a) of California Code of Regulation. Please submit proof of third-party <br /> approval. <br /> 5. Procedure or indication on how the stainless steel clamps will not be installed as part <br /> of the construction. <br /> 6. Monitoring and response plan. <br /> Please submit above information to me.
