EMPLOYEE TRAINING REQUIREMENTS v <br /> A.All personnel are trained in safety and emergency response <br /> procedures within six month of hiring. <br /> B.Refresher training is provided annually. <br /> C. All employees are trained and drilled in the following: <br /> I.Procedures for internal alarms and on-site notification of emergencies. <br /> 2. Procedures for emergency response notification off-site. / <br /> 3.Location and content of facility emergency response plan. ✓ <br /> 4.Evacuation and re-entry procedures. ✓ <br /> 5.Procedures for chemical spill mitigation. <br /> D.Hazardous material handlers are trained in the following: <br /> 1. Safe methods for storage and handling of theses of these materials. <br /> 2. Proper fitting and use of personal protective equipment. <br /> 3.Proper use and locations of fire and spill control equipment. / <br /> 4. Knowledge of specific hazards of each chemical to which employees <br /> may be exposed. Use the information available from material safety <br /> data sheets and any other relevant sources of information <br /> E.Emergency response team personnel are trained (addition to the above) <br /> in the following: <br /> 1. Rescue and containment procedures. <br /> 2.Procedures for shutdown of operations. <br /> 3. Use and maintenance of emergency equipment. <br /> VI. EMPLOYEE TRAINING DOCUMENTATION <br /> A. List the names and telephone numbers of facility personnel <br /> Responsible for training employees. <br /> B.Training records must include the following: <br /> 1.Training verification(i.e. date and signatures) <br /> 2. Description of introductory and continuing training <br /> (e.g.type and amount). <br /> 3. Training records to be retained for three years(or longer <br /> based on other laws). <br /> 4. Documentation must be available on-site for review by <br /> CUPA personnel. ✓ <br /> Rev.8-30-00 <br />
