L'c>< <br /> Business Nam Facility ID # <br /> Business Site Address <br /> Emergency response plans and procedures and an employee training program are required to be reported by <br /> hazardous materials handlers. These documents must be part of the Business Plan. The outline below is <br /> intended to serve as a template or check sheet so that any business that is required to produce and maintain <br /> these documents will have a beginning reference. The page # refers to that page number in your Business <br /> Plan that contains the item in the check sheet. Larger, more complex facilities should summarize these <br /> programs by documenting those personnel responsible for these plans and how they can be contacted; they <br /> must also provide a summary of the plans structure and where the more detailed programs are located at the <br /> facilities for review by CUPA personnel. <br /> I. NOTIFICATION/ALARM PROCEDURES PAGE NUMBER <br /> A. Call 911 �,O —53y--7)'f)' <br /> 1. Notify local fire department or public emergency response agency. <br /> 2. Notify medical emergency response. <br /> B. Notify on site response personnel. <br /> C.Activate local alarm systems. S. <br /> D. Notify Contra Costa Health Services/HazMat I.R.(925-646-1112). <br /> E.Notify State Office of Emergency Services(800-852-7550). <br /> (if the spill involves hazardous waste also notify Dept.of Health Svcs) <br /> F.Submit a written follow-up spill notification to the State O.E.S.and <br /> Contra Costa Health Services/HazMat. <br /> II. EVACUATION <br /> A.Procedures for evacuation which includes initiating the alarm; area, <br /> building and facility evacuations. <br /> B.Define routes of evacuations. <br /> C. Identify assembly areas. <br /> D. Identify where emergency response and evacuation plans are posted. v <br /> E.Procedures for re-entry. ;( <br /> III. EMERGENCY EQUIPMENT <br /> A.Describe emergency response equipment,its location and its use. i <br /> IV. EMERGENCY RESPONSE PROCEDURES <br /> A.Procedures for handling the release or threatened release for all <br /> hazardous materials listed on the chemical inventory <br /> B. List all emergency contact personnel.These people must have: <br /> 1.Technical knowledge and familiarity of that part of the facility <br /> that they are responsible for during an emergency. <br />
