Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Complete and submit a copy of the <br /> financial responsibility by July 20, 2011. <br /> 11. The 91/diesel tank annular sensor was raised approximately four inches off the lowest point of the <br /> tank bottom and was not located to detect a leak at the earliest opportunity. The tank riser was 174 <br /> inches and the sensor cord + sensor was 170 inches long. No liquid was detected in the tank annular <br /> space. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible <br /> opportunity. The testing technician lengthened the sensor cord and relocated this sensor to the lowest <br /> point of the 91/diesel tank annular space. <br /> 13. The vent sump liquid sensor failed to activate an audible and visual alarm when tested to ensure <br /> continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate an <br /> audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The sensor <br /> (Veeder Root part 794380-208, S/N 865648) was replaced and retested during the inspection. Obtain a <br /> permit from the EHD for this work within one business day. If a permit is not obtained within one <br /> business day, a "Failure to Obtain a Permit" penalty will apply. <br /> 22. Diesel fuel was observed under dispensers 1 and 15 and was not recorded in facility records. Any <br /> release from the primary containment that does not escape the secondary containment and was cleaned <br /> up within eight hours of being detected shall be recorded with the following information: <br /> 1. Operator's name and phone number <br /> 2. List of types, quantities, and concentrations of hazardous substances released <br /> 3. Description of actions taken to control and clean the release <br /> 4. Method and location of disposal of the released substances <br /> 5. Description of actions taken to repair and prevent future releases <br /> 6. Description of method used to reactivate interstitial monitoring after repair or replacement of <br /> primary containment <br /> Immediately record this release with the information required, and maintain it on site with the facility's <br /> monitoring records.
