We request that you proceed with the work plan with the <br /> following amendments: <br /> 1. Provide a site map with the locations of the four wells <br /> plotted. <br /> 2 . Secure encroachment agreements for all off-site wells and <br /> submit copies to San Joaquin County Public Health <br /> Services, Environmental Health Division (PHS/EHD) . <br /> 3 . Submit the well permit applications with the appropriate <br /> permit fee. <br /> 4 . Submit in writing a date for completion of the <br /> Preliminary Assessment Report.
