Barton <br /> CENTRAL VALLEY REGIONAL WATER QUALITY CONTROL BOARD <br /> 3443 Routier Road, Suite A <br /> Sacramento, CA 95827-3098 <br /> Subject: Ozone Injection Well Installation Report <br /> GEWEKE PROPERTY <br /> 16 South Cherokee Lane, Lodi, California <br /> Dear Mr. Barton: <br /> At the request of Mr. Jim Farris of G-REM,and in accordance with a letter dated 08 July 2005 from <br /> the Central Valley Regional Water Quality Control Board,Advanced GeoEnvironmental,Inc.(AGE) <br /> has installed additional ozone injection wells at 16 South Cherokee Lane, Lodi, California. The <br /> location of the site is illustrated on Figure 1. A site plan is illustrated on Figure 2. <br /> SCOPE OF WORK <br /> The scope of the work included the installation of three additional ozone injection wells (OW-4, <br /> OW-5 and OW-6) as well as the re-installation of ozone injection well OW-1R. A fifth ozone <br /> injection well,proposed west of the site across Cherokee Lane, could not be installed at this time <br /> because of logistical issues. <br /> Procedures for the installation of additional ozone injection wells were presented in AGE's Ozone <br /> Injection Pilot Study Work Plan, dated 15 December 2003. As discussed in the 08 July 2005 letter, <br /> an additional work plan for the installation of the ozone wells was not necessary. <br /> PERMITTING AND PRE-FIELD WORK ACTIVITIES <br /> Prior to initiation of drilling activities,AGE submitted a Well Permit Application and associated fee <br /> to the San Joaquin County Environmental Health Department(SJCEHD).The permit(SR#0043761) <br /> was approved by the SJCEHD on 01 September 2005.
