IN REPLY REFER TO SITE CODE: 1841 <br /> 900 S. Cherokee <br /> Lodi, CA 95240 <br /> We have reviewed the September 19, 1991, Quarterly Groundwater <br /> Report for the above referenced site. High levels of hydrocarbon <br /> contamination are present in soil and groundwater beneath this <br /> facility; however, the extent of groundwater contamination is not <br /> yet known. A vapor extraction system is in operation and appears <br /> to be effectively removing hydrocarbons from the vadose zone. <br /> Pursuant to Section 2652 (d) of the Underground Storage Tank <br /> Regulations (California Code of Regulations, Title 23, Subchapter <br /> 16) quarterly monitoring reports shall be submitted to Public <br /> Health Services, Environmental Health Division (PHS/EHD) and <br /> Central Valley Regional Water Quality Control Board (CVRWQCB) until <br /> the investigation and cleanup of the site is deemed adequate. The <br /> reports are to show the following: <br /> 1. status of investigation and cleanup activities including <br /> the results of all investigations completed to date; <br /> 2 . proposed continuing or next phase of investigation; <br /> 3 . method of cleanup proposed or implemented, schedules for <br /> the completion of the investigation of the site and <br /> remediation; <br /> 4. method and location of disposal of the released <br /> hazardous substance and any contaminated soil, <br /> groundwater or surface water; <br /> 5. manifest required for transport of hazardous <br /> substances; <br /> 6. monitoring well data including groundwater elevation, <br /> pH, conductivity, gradient;- <br /> 7. analytical results of groundwater or soil sampling; and <br /> 8. site map showing the "zero line" of contamination, if <br /> known, and changes in the above measurements over the <br /> last quarter. <br /> A Division of San Joaquin County Hculth Care Sesices <br />
