SITE CODE: 1536 <br /> 1645 Cherokee Lane <br /> Stockton CA 95205 <br /> In July and August 2000 six soil borings and three groundwater monitoring wells were <br /> installed at the above referenced site. The work was performed in accordance with the <br /> work plan titled "Work Plan Phase II Environmental Site Assessment"dated June 9, <br /> 2000, that was prepared by Hydro-Environmental Technologies, Inc. The work plan had <br /> been approved by San Joaquin County Public Health Services, Environmental Health <br /> Division (PHS/EHD) in correspondence dated June 15, 2000. The newly installed <br /> monitoring wells were scheduled to be developed and sampled on August 31, 2000. <br /> As a condition of the work plan approval, a report of findings was due to be submitted to <br /> PHS/EHD within 60 days following completion of all fieldwork. To date, this office has <br /> not received this report. The report is now almost five months over due. In addition, two <br /> quarters of groundwater monitoring have not been completed. <br /> Submit the report of findings for the work completed at this site in August 2000 to <br /> PHS/EHD immediately.
