APPENDIX B <br /> HEALTH AND SAFETY PLAN <br /> The purpose of the Health and Safety plan is to establish personnel training and personal <br /> protective equipment(PPE)requirements, physical and chemical hazards, and emergency <br /> response procedures for field personnel at the groundwater monitoring well installation <br /> site. All work will be conducted in conformance with the regulations and guidelines <br /> established by the General Occupational Safety and Health Administration (OSHA) and <br /> Cal-OSHA. <br /> A. PERSONNEL TRAINING <br /> The well drilling contractor will hold a valid Class C-57 California Contractor's license. <br /> All drilling contractor employees onsite will be experienced in the field of monitoring <br /> well installation. Additionally, subcontractor employees working onsite will be required <br /> to attend a site-specific health and safety"tailgate"orientation the first day at the site, and <br /> any time thereafter when site conditions change significantly. <br /> B. PERSONAL PROTECTIVE EQUIPMENT <br /> The following PPE will be required during field activities: <br /> • Hardhat at all times. <br /> • Safety goggles or safety glasses with side shields at all times. <br /> • Ear protection during drilling. <br /> • Steel-toed boots at all times. <br /> • Nitrile gloves during sampling. <br /> These PPE precautions and standard engineering controls should provide the necessary <br /> protection for onsite personnel. <br /> C.
