Ns <br /> 1 <br /> 522-0862 . The updating of the plan Was requested in December of <br /> 1990. A blank form is enclosed for your use in updating your plan. <br /> 5. The Depot must apply for a revision of its water supply permit <br /> since Well No. . 3 has been destroyed and is no longer a source of <br /> water for the Depot system. <br /> At the time of the inspection, Mr. Zulueta provided copies of some more ; <br /> recent general mineral , general physical, and inorganic chemical <br /> monitoring results. A review of the reports indicates that the re-- <br /> are <br /> esare not being reported in the format required by the Department. The <br /> Department has provided all laboratories in the State that are <br /> accredited potable water analysis laboratories with the acceptable <br /> format and a software package that will computer generate analytical <br /> reports that comply with the required office of Drinking Water format. <br /> The laboratory that performed the analyses in question has the ability <br /> to generate reports in the approved format," however, the customer must <br /> notify the laboratory that the reports are to be submitted to the <br /> Office of DrinkingWater and must be reported in the Department's <br /> approved format. Your agency may accept reports in any format and <br /> transfer the results to forms approved by the Department if you prefer <br /> that approach. Copies of the forms approved by the Department are <br /> enclosed. You may copy the forms as necessary for use in reporting <br /> analytical results to the Department <br /> Several of the deficiencies noted this year are the same deficiencies <br /> brought to the attention of your staff last year. Therefore, if your <br /> agency did respond to these deficiencies last year, it is evident that <br /> the information was lost in transmittal. Please respond to the <br /> deficiencies in writing by December 6, 1991.
