I 1 <br /> does not indicate how excessive plant shut down will be avoided in the <br /> future, e.g. , how the communication errors between the extraction wells <br /> and the plant will be remedied. To ensure that treatment plant #2 will <br /> be operating at close to 100% of the time, please indicate how excessive <br /> plant shut down will be avoided in the future. <br /> 4 . Appendix B, Treatment Plant Information: The table in the text states <br /> that between the 18t and 2nd of March 2000, "Plant shut down for 33.5 <br /> hours because plant was brought back on line after going down on <br /> December 16, 1999." Please clarify how the plant was shut down why <br /> bringing the plant back on line cause 33.5 hours of shut down. <br /> 5. Appendix B, Treatment Plant Information: The table indicates that pump <br /> failure caused numerous pumps to be off-line during the reporting <br /> period. Since during 4Q99 pump failure had also been a problem, please <br /> revise the Report to indicate how pump failure will be remedied in the <br /> future and what the time frame is for achieving proper operation of the <br /> pumps in the affected extraction wells. <br /> 6. Appendix B, Table 4-3: The footnotes of this table identify that flow <br /> meters on extraction wells EW3, EW6, EW9, and EW11 are not reading <br /> accurately. The Report provides no indication that any corrective action <br /> was performed for the inaccurate flow meters. The Report should <br /> document all corrective action for faulty flow meters within the <br /> maintenance notes section provided Appendix B. Therefore, please revise <br /> the Report to provide the requested information (Note: the same comment <br /> was made regarding the 4Q99 Monitoring Report) . <br /> 7. Appendix B, Table 4-4: The table shows that there was flow in extraction <br /> wells EW15 and EW22 during the month of March 2000. However, in the text <br /> (Page B-3) of Appendix B, it is stated that those wells were off-line <br /> March 2 through the 31, 2000 due to pump failure alarms. Please clarify <br /> this discrepancy. <br /> 3 <br />
