The "Installation Data Report" containing a record of the ground water cleanup <br /> system as installed, including completed installation check lists for each <br /> component and the results of the hydrostatic tests conducted on the systems' <br /> pipelines. <br /> c. The "Site Specific Quality Management Plan" (SSQMP) containing Quality <br /> Assurance and Quality Control (QA/QC) procedures involved in the collection, <br /> preservation, packaging, and transport of samples, field testing, laboratory <br /> analyses (including the mobile laboratory QA/QC procedures), record keeping, <br /> data management, and chain-of-custody. <br /> d. Authorized factory representatives' certifications that OU#1 components are <br /> properly installed. <br /> e. A "Start-up Schedule" for the start-up of the second phase of the full scale ground <br /> water cleanup system. The schedule shall include a start-up monitoring proposal. <br /> Full-scale operation shall not commence until the Executive Officer determines that the <br /> proposed system is capable of adequately treating all identified pollutants, that the <br /> system is capable of complying with waste discharge requirements, that the air stripper <br /> is capable of being operated to maximize VOC removal efficiencies, and that its <br /> continued operation is appropriate. <br /> 4. The Discharger shall submit a Technical Report to the Board and to the Department, <br /> prior to the end of the fourth month of operation of the new air stripper, which includes <br /> an analysis of the waste characterization data collected during the first three month <br /> operational period. This report shall include a summary of the data and a <br /> determination if the influent and/or effluent to the air stripper contains previously <br /> unidentified constituents of sufficient magnitude to either pose a threat to water quality <br /> and/or affect the treatment effectiveness of the air stripper. In addition, in the event that <br /> significant levels of previously unidentified pollutants are detected, this report shall <br /> include recommendations on the continued operation of this ground water treatment <br /> system, possible treatment plant modifications, and an assessment of the impacts to the <br /> beneficial uses of the receiving water due to this discharge. <br /> 5. The Discharger shall notify the Board and the Department, within 24 hours, of any <br /> shutdown of the full-scale ground water extraction and treatment system. This <br /> notification shall include the cause of the shutdown and the corrective action taken(or <br /> proposed to be taken) to restart the system. <br />
