A facilities inspection report and photograph log is attached. The system appeared to be well <br /> operated and maintained. <br /> During the inspection, I was informed that Crossroads CREA LLC has sold its ownership interest in the <br /> facility and land application areas. Waste Discharge Requirements (WDRs) Order No. 5-01-251, <br /> Provision F.13 requires notification of any ownership change to the Regional Board. By 9 July 2004, <br /> the new owner shall submit a completed Form 200, notifying the Regional Board of the new responsible <br /> party.
