KLEINFEL DER <br /> Task 4 Prepare Report of Findings <br /> Upon completion of Kleinfelder's services a report will be prepared for client review. The report <br /> will include: <br /> • A description of field activities, observations, and protocols; <br /> • Lithologic logs; <br /> • Results of the sample analysis with copies of certified analytical laboratory results <br /> with chain-of-custody documentation(if applicable); <br /> • Description and documentation of QA/QC procedures employed for field and <br /> laboratory work (if applicable); and <br /> CLIENT RESPONSIBILITIES AND ASSUMPTIONS <br /> This proposal and the attached cost estimate are based on the following assumptions and <br /> responsibilities: <br /> • Crossroads Commerce Center will supply Kleinfelder with access to the site and written <br /> owner authorization for the proposed services. <br /> • This proposal will be adequate to obtain authorization from PHS/EHD and the client. If <br /> additional workplan preparation is required, additional fees will be incurred. <br /> • PHS/EHD will bill Crossroads Commerce Center, Inc. for any oversight fees incurred above <br /> the initial 3-hour minimum and permit fee to be submitted by Kleinfelder. These fees will be <br /> billed directly to Crossroads Commerce Center by the PHS/EHD. Kleinfelder will forward <br /> the appropriate "billing and compliance acknowledgement forms" to Crossroads Commerce <br /> Center for signature. Signed originals of these forms must be submitted to PHS/EHD as part <br /> of the permit process. Our proposed schedule assumes that Crossroads Commerce Center <br /> will expedite the completion of these forms and their return to Kleinfelder for submittal to <br /> PHS/EHD. <br /> • Adequate workspace and access will be provided. Kleinfelder will attempt to advance soil <br /> borings and collect samples at the proposed locations. If the drilling equipment cannot be <br /> positioned, alternative drilling locations or sampling methods should be investigated. <br /> • The borings are to be drilled with a truck mounted drill rig equipped with hollow stem <br /> augers.
