Please provide me with paperwork, photos, or a statement of correction for the training record <br /> violation and a signed Return to Compliance Certification found on last page of inspection report. <br /> A training record is a document that shows that an employee was trained the items listed in California HSC <br /> 25505 (a)(4). The training records on site need to state the topics covered, date of training, and name of <br /> employees trained. A copy of the training records must be kept on site for a minimum of three years and be <br /> made readily available for review upon request per California HSC §25505(a)(4). Employee training must be <br /> refreshed annually and by the anniversary date. For example, if the employee training was done in May 2018, it <br /> needs to be refreshed by May 2019.
