This is a Class II violation. 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. The Hazardous Materials Inventory forms for diesel fuel, LPG, used oil, acetylene, oxygen and motor oil are not complete/accurate. 1. The size of the largest container, average daily amount, and days on site data elements have not been completed for diesel and compressed oxygen. 2. The size of the largest container and average daily amount data elements have not been completed for LPG, acetylene, and motor oil. 3. The days on site, storage pressure, and storage temperature data elements for the used oil have not been completed, and the maximum daily amount of used oil is wrongly identified as 110 gallons vs. the 220 gallon (four 55 gallon drums) observed at the time of inspection. This information must be current and submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. The map does not show/address loading areas, internal roads, storm and sewer drains, access and exit points (structures), emergency shutoffs, evacuation staging areas, and the actual storage locations of the diesel, waste oil, compressed oxygen and acetylene. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA". Immediately log into the California Environmental Reporting System(CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. The business plan information has not been reviewed and resubmitted in the California Environmental Reporting System(CERS)annually for 2019. The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year.
