Time In: 12:58 pm <br /> Time Out: 1:43 om <br /> Food Program Complaint Inspection Report <br /> Name of Facility: LA SUPERIOR WATERLOO INC Date: 02/21/2019 <br /> Address: 1536 WATERLOO RD, STOCKTON 95205 <br /> Owner/Operator: LA SUPERIOR WATERLOO INC Telephone: (209)546-7950 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00048926 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES RESTROOMS ARE FILTHY.THEY HAVE WATER AND TOILET WATER ALL OVER THE <br /> FLOOR. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Upon arrival, spoke with manager who states having no knowledge of complaint. Manager does state that there is an issue <br /> with the homeless population that use the restroom to shower themselves or misuse it. Upon observation, restrooms have <br /> small amount of trash on the floor but observed no toilet water on floor or on toilet seats. Manager states there is a bathroom <br /> log that is kept and every hour someone scans a bar code when they are done cleaning the restroom.
