Commerce Street <br /> Stockton, CA <br /> In a letter dated June 14, 1995, San Joaquin County Public Health <br /> Services, Environmental Health Division (PHS/EHD) directed you to <br /> submit a work plan to continue the investigation of the extent of <br /> soil contamination and to initiate a groundwater investigation at <br /> the above referenced site by August 1, 1995 . This work plan was <br /> also to include sampling and analysis of the remaining stockpiled <br /> soil at 1111 N. Union Street, Stockton, so that decisions on its <br /> disposition could be evaluated. <br /> In that same letter PHS/EHD directed you to submit a report of <br /> all past investigations at 729 S . Commerce Street, Stockton, with <br /> soil sample results and supporting original lab data, prepared by <br /> a properly licensed individual in the State of California by <br /> August 1, 1995 . PHS/EHD has not received either of these <br /> documents to date . <br /> PHS/EHD has been informed by State Water Resources Control Board <br /> Underground Storage Tank Cleanup Fund (Cleanup Fund) staff that <br /> as long as a death certificate and estate information is <br /> submitted to them the Letter of Commitment issued to F. D. Clarke <br /> on November 9, 1994 by their office can be utilized by the <br /> estate . <br /> PHS/EHD has a copy of a letter from the Cleanup Fund that denied <br /> pre-approval for work plan costs submitted to them on your behalf <br /> by Neumiller and Beardslee . PHS/EHD was informed by Cleanup Fund <br /> staff that the costs submitted included work that had already <br /> been performed and that pre-approval is only for work that has <br /> not been performed yet . In addition, it was suggested that <br /> project costs appeared high so three consultants bids should be <br /> obtained for the work. <br /> PHS/EHD has found that when responsible parties are actively <br /> involved in the decisions made regarding their sites that work <br /> A Division of San Joaquin County Health Care Services <br />
