MONITORING AND REPORTING PROGRAM -2- <br /> SANTA FE PACIFIC PIPELINE PARTNERS, INC. <br /> HOLT PIPELINE LEAK <br /> SAN JOAQUIN COUNTY <br /> REPORTING <br /> Quarterly reports shall be submitted to the Board by the 30th day of the month following the <br /> end of each calendar quarter until such time that the Executive Officer determines that the <br /> reports are no longer necessary. These reports shall include the following in tabular form: <br /> water quality analytical data, depth to ground water, free product thickness, and ground <br /> water elevations. In addition, the reports shall include ground water contour maps for each <br /> quarter. <br /> Prior to startup of a ground water treatment system, the Discharger shall supply Board staff <br /> with a ground water monitoring program that°will demonstrate the effectiveness of the <br /> ground water treatment system. <br /> An annual report evaluating the effectiveness and progress of investigation and remediation <br /> shall be submitted to the Board by 30 January of each year and may be substituted for the <br /> fourth quarterly report. The report shall contain at a minimum the following information: <br /> both tabular and graphical summaries of the monitoring data obtained during the previous <br /> year; trends in the concentrations of the pollutants in ground water monitoring wells; whether <br /> the contaminant plume is being captured by an extraction system or is continuing to spread; <br /> plans as needed for improving the ground water extraction or treatment system; and the <br /> anticipated date for completion of cleanup activities. The annual report also shall include the <br /> reasons for, and durations of, all interruptions in the operation of treatment systems. <br /> Based on the data from the previous year, the Discharger shall, in the annual report, identify <br /> data gaps and potential deficiencies/redundancies in the monitoring system or reporting <br /> program and propose any necessary revised ground water sampling plan frequency and <br /> analyses with the rationale for the proposed revisions. <br /> The results of any monitoring done more frequently than required at the locations specified in <br /> the Monitoring and Reporting Program shall be reported to the Board. <br /> The Discharger shall implement the above monitoring program as of the date of this Order. <br /> WILLIAM H.
