WASTE DISCHARGE REQUIREMENTS -6- UNIVERSITY OF CALIFORNIA LAWRENCE LIVERMORE NATIONAL LABORATORY SITE 300 AND THE U.S. DEPARTMENT OF ENERGY ALAMEDA AND SAN JOAQUIN COUNTIES 3. Within 30 days of the adoption of this Order, the discharger shall submit for the approval of the Executive Officer a GWTS evaluation program for the eastern GSA which shall describe, but not be limited to, the following: a. the monitoring wells chosen to assess the effectiveness of the ground water cleanup, b. the extraction well ' s radius of influence, as determined through appropriate pump tests, C. the proposed sampling frequency and constituents to be analyzed. 4. The Discharger shall comply with the "Standard Provisions and Reporting Requirements (NPDES)" , dated 1 October 1990, which are part of this Order. 5. The Discharger may be required to submit technical reports as directed by the Executive Officer. 6. The Discharger shall comply with the attached Monitoring and Reporting Program No. 91-052 as ordered by the Executive Officer. 7. This Order expires 'on 1 February 1996 and the Discharger must file a Report of Waste Discharge in accordance with Title 23, California Code of Regulations, not later than 180 days in advance of such date as application for issuance of new waste discharge requirements. 8. In the event of any change in control or ownership of land or waste discharge facilities presently owned or controlled by the Discharger, the Discharger shall notify the succeeding owner or operator of the existence of this Order by letter, a copy of which shall be forwarded to this office.
