CORRECTIVE ACTION REPORT <br /> (834-D1U1) <br /> On July 27, 1994, the 1000-gallon steel underground diesel fuel tank 834-D1U1, <br /> identifed by San Joaquin County as 39-1945-12, at Lawrence Livermore National <br /> Laboratory Site 300 Building 834 was removed from the ground and shipped off- <br /> site as hazardous waste. Bill Snavely of the San Joaquin County Environmental <br /> Health Division (SJC-EHD) was present to witness closure activities and the <br /> collection of soil samples. After removing the tank we became aware that the <br /> underlying gravel fill and soil smelled of diesel fuel. Excavation was stopped at <br /> a depth of 13 feet because of the proximity to the underlying perched aquifer. <br /> Soil samples were collected from the bottom of each end of the excavation at a <br /> depth of 13 feet. The samples were submitted for analyses of diesel fuel (TPH-D) <br /> and BTXE. Laboratory analytical results and chain of custody records for the <br /> environmental samples are enclosed as Appendix E. <br /> Groundwater was measured to be 17 feet 8 inches below ground surface in the <br /> water monitoring well _ <br /> group g <br /> An Unauthorized Release Report was completed and both FAXed and mailed to <br /> San Joaquin County Public Health Services on August 3, 1994. A copy of the <br /> release report is enclosed at Appendix G. <br /> Bill Snavely stated a belief that the diesel fuel evident below the tank was <br /> associated with the previously identified diesel fuel in groundwater samples <br /> collected from well W-834-D8, and that both have the underground diesel fuel <br /> tank as their source. This lead Mr. Snavely to transfer the closure site to Ms. <br /> Diane Hinson, Supervisor, Site Mitigation / Local Oversight Program for <br /> investigation. <br /> The results of analyses of soil samples collected from a depth of 13 feet at the <br /> bottom of each end of the tank excavation showed 6900 mg/kg of diesel fuel <br /> (TPH-D) and 0.39 mg/kg of xylene (BTXE) in soil from the west end, and 410 <br /> mg/kg of diesel fuel and no BTXE detected in the east end sample. <br /> On August 1, 1994, the 834-D1U1 tank closure project was discussed with Diane <br /> Hinson, after having been transferred by Bill Snavely. Ms. Hinson was first <br /> notified of the 834-D1U1 tank closure project on March 4, 1994. On March 4, it <br /> was noted that the upcoming closure of the 834-D1U1 underground diesel fuel <br /> tank system might reveal a release that would explain the small concentrations <br /> of diesel fuel detected in water samples collected from well W-834-D8.
