I didn't know this plan when I <br /> discussed the fees for permits back in August. <br /> After discussing this change from normal working hours with my boss, it was decided that <br /> premium hourly fees (OT @ 1.5 time) would be charged for all borings, permits, inspections in <br /> place of the normal fees. <br /> Normal fees are based on a $122 per hour and 'per' well fee. This fee will be adjusted to the <br /> 'time-and-a-half rate, that is $183 per hour, per well, per permit. Instead of the three hour rate <br /> at $122 per hour ($366), the adjusted fee is now 3 hours at $183. ($549). So simply put, we <br /> will require the difference between the two checks you wrote to be additionally added to <br /> to reflect those charges at a 1.5 rate. <br /> The fee for the onsite soil vapor wells and CPTs permit was determined now to be $1281. (1.5 <br /> times the $854 check #20350 you submitted). You need to send in 427 more for the <br /> ONSITE permit. <br /> and the permit fee for the OFFSITE CPT (#10) is now $732. (1.5 times the $488 on check <br /> #20351 you submitted), or$2244 more. <br /> You can send in ONE check for 671 if you'd like. I will be able to issue both permits after I <br /> receive this amount. <br /> Please include a recognition statement to the EHD that you/CRA understands and agrees <br /> that any additional inspection fees over and above these amounts will charged to CRA (billing <br /> party identified on our MFR form you submitted with the permit applications) and are due at the <br /> EHD within 30 days of the date of the billing statement we send to CRA. <br /> with these items adequately addressed by October 13, 1 will be able to issue the two permits <br /> and you can be able to meet your schedule for the first day for this phase, Sunday,October 17. <br /> As I explained to Dana, the EHD will inspect soil vapor probe installations and surface seal <br /> testing, CPT advancement and sample collection, as well as CPT grouting/destruction <br /> procedures. On Sundays these activities are planned, the EHD staff inspector is to be called <br /> prior to the work being done. The EHD does not need to inspect the air knifing/hole clearing <br /> procedures. Please keep me informed of the work you plan each Sunday as soon as you have <br /> determined it.
