04 August 2009 <br /> AGE-NC Project No. 98-0465 <br /> Page 2 of 2 <br /> Drilling&Testing, Inc.using a CME-75 drill rig equipped and 8.25-inch/10.25-inch hollow stem <br /> augers. Prior to well destruction,each well casing was sounded with a Solinst water level meter to <br /> ensure that no obstructions were present in the casings.Thereafter,the drill rig hoist was utilized to <br /> remove each monitoring well box steel ring. <br /> Monitoring wells MW-1,MW-2 and MW-5 were destroyed at the site by connecting a cable to the <br /> top of the well casing and removing the entire well casing with the drill rig hoist.Thereafter,the well <br /> casing void space was backfilled utilizing portland type II grout. Four 50-pound bags of portland <br /> type II cement were utilized to backfill the well casing void space;a ratio of three gallons water for <br /> every 50-pound sack of portland cement was utilized in preparation of the grout mixture. Each well <br /> was completely backfilled to surface grade with the portland cement grout mixture. <br /> Monitoring wells MW-3 and MW-4 were over-drilled to total depths of 25 feet bsg due to the <br /> inability to remove the well casings with the drill rig hoist. Following each over-drill, the well <br /> casings from MW-3 and MW-4 were removed and inspected by a representative of EHD. Total <br /> depths of over-drilled borings MW-3 and MW-4 were measured at 25 feet bsg;thereafter,each over- <br /> drilled boring was completely backfilled with a portland type 11 cement grout mixture.
