CUF#: 014052 <br /> San Joaquin County Environmental Health Department(EHD) has received written concurrence for the <br /> No Further Action (NFA)request submitted to the Central Valley Region Water Quality Control Board <br /> (CVRWQCB). Conditions included within the NFA concurrence called for the monitoring wells to be <br /> properly addressed before the site can be considered closed. <br /> In order to finalize the NFA, you are to comply with one of the following options and notify EHD: <br /> • submit a work plan to destroy the wells or, <br /> • submit documentation that the monitoring wells have been legally transferred to another <br /> responsible party or, <br /> • submit notification to EHD that you plan to keep the wells and continue to monitor and <br /> sample them on a quarterly basis. <br /> If you plan on destroying the wells, be advised that monitoring wells with detectable concentrations of <br /> petroleum in the groundwater may only be destroyed by removing all of the contents of the well (over- <br /> boring). Monitoring wells that are non-detect for petroleum may be destroyed by pressure grouting <br /> (complete with the"mushroom cap"at the surface) as detailed in San Joaquin County Well Standards, <br /> or over-bored. Please submit a well permit application form and $60 application fee per parcel along <br /> with the work plan if this is your choice. Offsite monitoring well "KF-7"will additionally require a <br /> private parcel access agreement to be on file with EHD before the destruction permit can be issued. <br /> For monitoring wells that are to be transferred to another responsible party, documents that detail the <br /> provisions are to be on file with EHD before the final NFA can be issued. The sampling, maintenance, <br /> and eventual destruction of each well will be the responsibility of the new owner. <br /> If you plan to continue monitoring and sampling the wells, please advise EHD.
