ROM 0000484 <br /> Stockton , CA., 95204 <br /> V San Joaquin County Environmental Health Department (SJCEHD) reviewed September 2004 <br /> Self Monitoring Report dated October 11 , 2004, submitted by your consultant, Cambria <br /> Environmental Technology, Inc, and has the following comments. <br /> This report stated that " The system (ground water extraction from RW-1 A) was shutdown on <br /> August 5, 2004 and that no discharge of,ground water occurred during September 2004 and <br /> monthly self-monitoring samples were not collected for this period. " SJCEHD has checked our <br /> files and discovered that there has been no submittal of an August 2004 Self Monitoring Report. <br /> 11 <br /> In a phone conversation with Cynthia Vasko , Cambria Senior Staff Engineer on November 3, <br /> 2004 , SJCEHD learned that ground water extraction has not occurred at this site since the <br /> system shut down . <br /> As directed in SJCEHD correspondence dated April 4, 2004, Shell was to either continue to <br /> extract ground water or submit a revised Remedial Action Plan to modify the approved remedial <br /> action . No modification to the RAP has been received . <br /> This is you official notice that you are not in compliance with directives issued by SJCEHD. You <br /> have 14-days to resume ground water extraction or formal enforcement action may be requested <br /> by the San Joaquin County District Attorney's Office or the Central Valley Region Water Quality <br /> Control Board (CVRWQCB) . <br /> You may contact Michael Infurna at minfurnaosicehd .com if you have any questions.
