(Figures 1 and 2) The work is proposed by Fugro to investigate the extent of a gasoline release, <br /> which was discovered on December 7, 1994 <br /> The investigation is based in part on 1) information provided to Fugro by Shell, 2) <br /> communications between SJCPHS/EHD personnel and Fugro since December 7, 1994, and, 3) <br /> the results of ongoing soil and groundwater investigations and mitigation efforts performed on <br /> site since December 10, 1994 <br /> Background <br /> On December 7, 1994, the SJCPHS/EHD was notified by Shell of a suspected product loss from <br /> an underground regular-unleaded fuel storage tank at the site The loss was due to corrosion of <br /> the south regular unleaded underground storage tank (UST), which is part of two 5,000 gallon <br /> UST manifolded together The total release was estimated to be approximately 8,500 gallons <br /> During the afternoon of December 7, 1994, Shell requested that Fugro perform a reconnaissance <br /> of the site and vicinity to determine if there were any local receptors or conduits above or below- <br /> ground Fugro performed the reconnaissance on the evening of December 7, 1994, and did not <br /> observe any apparent receptors or conduits near the fuel tanks <br /> On December 8, 1994, Fugro personnel requested permission from Ms Margaret Lagono and Mr <br /> Doug Wilson of the SJCPHS/EHD to proceed with investigative/mitigation field work on site <br /> 94478620 AWP A member of the Fugro group of companies with offices throughout the world <br />
