I've attached it for you. <br /> typically EACH application must have a signed MFR with it at <br /> time of submittal. One MFR can work for both applications IF <br /> the "Third Party Billing" (bottom section of the form) shows <br /> the same billing party. 99.99% of the time CONSULTANTS take on <br /> the third party billing for the field work phase. after the <br /> wells are destroyed and I sign off the permit as 'finalled', <br /> the MFR is destroyed. the third party will not receive any EHD <br /> billing for this phase after the final bill for the phase has <br /> been paid. <br /> I've attached a PDF copy of the MFR for you to complete and <br /> return to me so I can process your applics. <br /> the EHD charges a ONE HOUR fee for each well, the $125 per you <br /> submitted.
